I’m planning to Do a Emergency hand brake adjustment on my 1987 Alfa Romeo Spider Graduate. I’m assuming this requires adjusting the cable from under the car. Ant tips on adjusting would be appreciated.

Thank you, very helpful. Do you happen to know if I can do an adjustment just with the cable under the car or have to go into the shoe housing?

To properly adjust you need to do both. It's pretty easy.

1) Loosen the cable adjuster to get some slack (make sure the handle is down)
2) Adjust the toothed wheel with a flathead screwdriver until you can't turn the axle any more, then loosen it a few notches until the axle turns freely. There's a hole in the rear brake disk/hub you do this through, just rotate the disc until you can see the adjuster.
3) Retighten the cable adjuster to just take up the slack
