Serious alignment issues evident in the photos. For a big start, you're missing the rear engine mount spacers between the mount and frame.That is why you needed the washer stackup to make it work.

Josh the shop manuals I have don't list the height of the collar, the only way for me to determine it would be for me lift my car and crawl underneath and measure one but that's not possible today. I would guess they are about 12mm; if you were to use one nut and a washer between the mount and frame that would be an improvement over the setup you have now until you can source them.
Regarding your alignment problem along the longitudinal axis of the car, can you take photos of your De Dion tube where it mounts to the car, front and rear, including the Watts linkages, your tranmission mounts, and post them?

Regarding your noise. Have you carefully looked for signs of metal-to-metal contact all along the drive line? I recommend you crawl under the car and inspect all eight flywheel mounting bolts. If one has vibrated loose and backed out its head can contact the front flex coupling mounting studs that are pressed through the flywheel as the prop rotates. This can shear the head off the bolt, then you're in for some real time and money. You'll need a flashlight and a 17mm wrench to rotate the prop shaft.

Josh I realize camera angles can fool one so please confirm or deny what I think I'm seeing here. In the picture below, my diffferent color lines represent the following: 1. Red vertical lines: Is the front crossmember bolt actually canted that far off from vertical? center bolt is red-lined as a point of comparison. 2. Blue curved line: is the front mount straight until passing the gap then curving upwards? This would explan the orientation of the bolt. 3. Green Z line: is the mount crossmember actually below the front chassis? 4. Gold horizontal I: is that gap actually as wide as it appears, and is the gap the same on the other side of the car, which appears to have similar issues.

I ask because as compared to my Alfetta GT: 1. all three bolts are perpendicular to the longitudinal axis of the car as your back two appear to be; 2. the mount they pass through is straight and level; 3. the bottom of the front crossmember mount is flush with the bottom of the chassis when bolted up snug; and 4. the gap between crossmember mount and chassis is nowhere near that wide.

Also, in looking at your rear suspension, have you carefully inspected the Watts pivot bush and end link bushes? There should be no metal-to-metal contact anywhere and all rubber should be intact.

Yup. All of this may just be camera angle and shadow, even if it's not it still may not be relevant to his problem, it just caught my eye. Then when I looked at the center support bearing photo with its obvious down-and-to-the rear pull, I had to ask.

Josh before you buy or replace anything can you determine why it is the way it is? Or is there a local mechanic you trust that can examine the car? My concern is someone purposely installed it that way because of a misalignment or damage elsewhere on the car.
